Should you TEST FOR TRICHOMONAS?
- You have an unusual discharge, particularly frothy, yellow green or with a strong smell.
- You have genital itching, soreness or swelling.
- You have discomfort passing urine or during sex.
- You have been treated for thrush or bacterial vaginosis and the symptoms have not resolved. This is one of the commonest routes to a missed trichomonas diagnosis.
- A partner has tested positive, even if you have no symptoms.
- You have had unprotected sex with a new partner.
- You are pregnant and have discharge symptoms, since trichomoniasis in pregnancy is associated with preterm birth and warrants treatment.
WHAT IS THE TRICHOMONAS TEST?
Trichomoniasis is caused by Trichomonas vaginalis, a single celled parasite passed on through unprotected vaginal sex and shared sex toys. Despite being the most common curable STI in the world, it is comparatively under diagnosed in the UK, largely because it is not routinely included in standard testing.
Symptoms differ sharply between men and women. In women it commonly causes a discharge that may be frothy and yellow green with an unpleasant smell, along with itching, soreness, swelling and discomfort passing urine or during sex. In men it usually causes nothing at all, or at most mild urethral discomfort or discharge. That asymmetry is why it persists in couples: one partner has symptoms and gets treated, the other has none, is not treated, and reinfects them.
Why it gets missed. The symptoms overlap almost completely with bacterial vaginosis and thrush, both far more common and both treated presumptively. So a pattern emerges of repeated thrush treatments that do not work, or bacterial vaginosis that keeps returning. If that is your experience, asking specifically for a trichomonas test is reasonable.
In pregnancy it is worth treating, since trichomoniasis is associated with preterm birth and low birth weight.
Treatment is straightforward. A short course of metronidazole clears it in the great majority of cases. Both partners must be treated at the same time, and alcohol should be avoided during the course and for a period afterwards because of an unpleasant interaction.
