Some people have ongoing urinary or genital symptoms, discomfort passing urine, discharge, pelvic pain, and have already tested negative for chlamydia and gonorrhoea. That situation is frustrating and it is where testing for Mycoplasma genitalium is genuinely useful, because it is a recognised cause of exactly those symptoms and is not included in most standard tests.
We should also be straightforward about the other side of this. If you have no symptoms, we would generally advise against testing for these organisms, and that is not us declining your money for no reason. Ureaplasma and Mycoplasma hominis are found in the genital tract of large numbers of healthy people with nothing wrong, so a positive result usually means you are normal rather than infected. UK guidance does not recommend looking for them, and treating them unnecessarily contributes to antibiotic resistance, which for Mycoplasma genitalium is already a real problem.

Particularly where chlamydia and gonorrhoea tests have come back negative. This is the clearest indication.

Should you BE TESTED?
Yes, worth testing if:
- You have persistent discomfort passing urine, discharge or pelvic pain, and chlamydia and gonorrhoea have come back negative.
- You have been treated for urethritis and the symptoms have not resolved.
- A partner has tested positive for Mycoplasma genitalium.
- You have unexplained bleeding between periods or after sex alongside other symptoms.
- You have pelvic pain being investigated and Mgen has not been excluded.
Probably not worth testing if:
- You have no symptoms at all and simply want the broadest possible screen. We will explain why, and what is worth testing instead.
MYCOPLASMA VS UREAPLASMA
Mycoplasma genitalium is a genuine sexually transmitted pathogen. It is a recognised cause of non-gonococcal urethritis in men and of cervicitis in women; it is associated with pelvic inflammatory disease, and it can cause exactly the persistent symptoms that people struggle to get explained. Testing it when there are symptoms is worthwhile.
Its complication is antibiotic resistance. Resistance to azithromycin, the drug that used to work well, is now common in the UK, which is why guidance recommends resistance testing where available and a sequential treatment approach rather than a single standard course. It is also why treating people who have no symptoms is actively unhelpful: it exposes the organism to antibiotics, drives resistance further, and offers the patient nothing.
Ureaplasma urealyticum and Mycoplasma hominis are a different matter. Both are found in the genital tract of large numbers of people who have no symptoms and no disease, and they are generally regarded as normal flora rather than infections. UK guidance does not recommend testing for them. If you test a hundred healthy people you will find these organisms in a substantial number of them, and telling those people they have an infection produces worry and antibiotics without any benefit.

Specialist NHS sexual health clinics are well placed for this, because Mgen with resistance testing and sequential treatment is genuinely specialist work. Private testing is worth paying for when you want to be seen quickly, or have found it hard to get persistent symptoms taken seriously.

Mycoplasma genitalium is associated with pelvic inflammatory disease, which can affect fertility. The evidence is less established than for chlamydia, and it is a reason to treat symptomatic infection properly rather than a reason to panic.
