What is Asthma Testing?
Asthma is airway inflammation that causes narrowing, and the narrowing is variable, which is the key to diagnosing it. Symptoms come and go, are often worse at night or early morning, and are set off by triggers such as exercise, cold air, pollen, dust or infection. That variability is what a good assessment looks for, and it is why a single normal test does not rule asthma out.
UK guidance changed in November 2024 with a joint NICE, BTS and SIGN guideline that restructured the diagnostic pathway. The important change was to move away from diagnosing on symptoms and a trial of inhalers, toward objective testing, with FeNO and spirometry as first line tests in adults and children over five. FeNO measures nitric oxide in your breath, which rises when the airways are inflamed in the way they are in asthma. Spirometry measures the obstruction itself, and reversibility testing shows whether it improves with a bronchodilator.
Why this matters to you practically: a diagnosis made properly is one you can rely on. Asthma is a long term condition with long term treatment, and both an incorrect label and a missed diagnosis carry real cost. If your symptoms turn out not to be asthma, that answer is just as valuable, because it redirects the investigation to what is actually causing them.
